Privacy Statement

 

Health Tribe Sydney is committed to providing you with the highest level of service. This includes protecting your privacy. The Privacy Act 1988 (Cth) sets out several principles concerning the protection of your personal information, with which we comply fully. We will only collect information that is necessary and relevant to provide you with quality health care and treatment, and to manage our practice.

 

Your personal information

 

Personal information held by Health Tribe Sydney may include your name, date of birth, current home address, home phone number, mobile phone number, email address, occupation and various medical reports, questionnaires, imaging, and notes.  We may also hold information connecting you to other patients and clients, who are you family members.  We may hold information in relation to legal matters between you and other patients and clients.

 

How we collect your personal information

 

We collect personal information in several ways, including:

  • Directly from you via phone, email, online or in documents such as the New Patient Form completed before your initial appointment.
  • From medical records and imaging supplied to us by your General Practitioner, Specialist, Hospital or Imaging Practice where applicable.
  • In communication as necessary with Private Health Funds, Insurance Companies, Disability Support Organisations and/or Government Organisations such as Medicare and NDIS where applicable.

 

How we use your personal information

 

Your personal information may be used for:

  • Assessment and Treatment during your appointment.
  • Disclosing information with your consent to your General Practitioner, Specialist Hospital or Imaging Practice.
  • Disclosing information with your consent to your Private Health Fund, Insurance Company, Disability Support Organisation and/or Government Organisations such as Medicare and NDIS where applicable.
  • When required and authorised by law, such as court subpoenas.
  • To lodge private health fund claims and/or Medicare rebates.
  • For practice management and marketing purposes.

 

Who can access your personal information

 

Client files and information, outside of our practice management software (see below), are not stored in the cloud, and can only be accessed from a local, encrypted and password protected database.  Practitioners and staff have limited access to this information depending on their role as practitioner, practice manager, or administrative staff.

We also use external administration software such as Cliniko and Physitrack to keep our practice running smoothly, and for your benefit.

All staff members and practitioners who treat at Health Tribe Sydney, have signed confidentiality agreements.

All staff members must adhere to this privacy policy when using your information to:

  • Answer personal queries about appointment date and time, referrals and clinical notes.
  • Answer general queries from third parties such as your referring General Practitioner, Specialist, Hospital, Imaging Practice, Private Health Fund, Insurance Company, Disability Support Organisation and/or Government Organisations such as Medicare and NDIS where applicable.
  • Ensure appropriate administration and security of your personal information such as scanning, filing and downloading to the secure database.

You also have the right to access your personal information. You may ask to view the information we have stored or ask for a copy of your record, which can be sent digitally or given to you in person.
